Location:McCart Neighborhood ClinicDepartment:McCart Neighborhood ClinicShift:First Shift (United States of America)Standard Weekly Hours:36Summary:Functions in a collaborative practice with physicians in a Neighborhood Health Center setting with a patient population of > 90% government payor or underinsured population. Autonomously performs patient assessment, diagnosis, and intervention according to approved protocols. Is able to evaluate and incorporate social determinants of health into treatment plan. Works alongside a multidisciplinary team to effectively support a high volume of socially and medically complex patients. Supports physician to meet empaneled patient demand and quality goals. Serves as a patient advocate by collaborating with others to connect families to needed resources. Prescribes a variety of modalities to meet wellness and illness needs. Serves as a resource for office clinical personnel as needed along with precepting new providers.
