About Us Do you have a penchant for organizing, collaborating, smiling, and having fun? We are a medical stabilization unit located in a prominent Dallas hospital. Our team provides support and medical care to those patients undergoing alcohol and drug withdrawals, among other medical complications. Position Summary We are currently seeking a Discharge Coordinator / Discharge Planner. The role involves continuous assessment of the patient's status and thorough documentation of pertinent information. Essencial Duties and Responsabilities Accurate documentation of communication and coordination for the patient's discharge and transition to the next level of care. The incumbent will serve as an information resource to patients and maintain consistent and effective communication with all departments within the unit. Responsible for matching resources and referrals to meet the needs of the patients. Education & Experience LCDCi Licensed Licensed Chemical Dependency Counselor (LCDC). One year or more of experience in discharge planning or supporting patients transitional care is preferred (but not required). Employment Type: Full Time
